I have searched the ads on here and have been unable to locate the model. I found the same watch face with a different band in the forum archives from 2012. Someone suggested "The First Lady".   The diamonds are real.  The glass is in excellent condition.

According to a local Bulova dealer it works.

It is a real beauty!  I looked through the ads and didn't an exact match yet.  I found a couple that were close, a La Petite, a Candlelight, and a First Lady.  There were so many diamond watch collections in the 60's.  Hopefully someone will find an ad.

Unknown for now

Sorry it doesn't show in the photo very well but it is stamped M5 on the movement with a  symbol that looks like a wide downward "v".

I've come back to this one a few times, but still find no match.

It's a beauty, but still unknown for now.
